The revised COPDOL 10 form, incorporating (in particular) the additional questions posed by Charles J in Re NRA, is to come into effect on 1 December.  A copy of the form is availablehere, but the 'embargo' marking is a reminder that it cannot be used before 1 December (the current form, in PDF and unofficial Word form) is availablehere.
